Parents & Guardians of Children entering Kindergarten: Does Your Child Have the Shots Needed to Start School?

To enter school, children shall have received the following:

  • One Varicella (chicken pox) vaccine if born on or after September 15, 1997, or a reliable history of the child already experiencing the disease

  • At least three doses of Hepatitis B vaccine if born on or after July 1, 1994.

  • At least four doses of combined diphtheria, tetanus, and pertussis vaccine if born after September 15, 2000; or 3 doses if born on or before September 15, 2000. (At least one dose of combined vaccine shall have been received after the child’s fourth birthday)

  • At least three doses of trivalent polio vaccine. (At least one dose of combined vaccine shall have been received after the child’s fourth birthday)

  • Two measles, mumps, and rubella (MMR) vaccines.
